GLP-1s Versus Phentermine Weight Loss

Hey everyone -
The question about phentermine comes up enough I thought it'd be good to write a quick note about it.
Short version: GLP-1 medications (Semaglutide and Tirzepatide) work on your metabolism and appetite signaling. Phentermine is a stimulant that suppresses appetite. One is built for the long haul. The other is a short-term jumpstart.
Here's the thing — they're not even solving the same problem.
GLP-1s mimic a hormone that regulates appetite, slows stomach emptying, and quiets down what we call "food noise." I remember my first week on this medication. The food noise just... went quiet. OMG. I didn't feel like I was on a diet. I felt like what I can only imagine "normal" people feel.
Phentermine is different. It's a stimulant. Suppresses hunger for a while. It's been around forever. Some people see an early drop. But the appetite suppression fades, and when you stop — a lot of people end up right back where they started.
Sound familiar? Yeah. That's the cycle.
Which one produces more meaningful weight loss? In most cases, GLP-1s. The data backs it up, and I see it in our patients every week at both our Puyallup and Federal Way locations.
Phentermine can give you a faster initial drop. I'll give it that. But sustainability? That's where the gap shows up. Big time.
And side effects matter. GLP-1s can cause nausea, especially when you titrate up — which is why we start low and go slow. Phentermine can cause jitters, racing heart, insomnia. Not a great fit if you already deal with stress or blood pressure issues.
Here's something else people don't always know — phentermine is only FDA-approved for short-term use. The label says "a few weeks," typically interpreted as up to 12 weeks (about 90 days) per course. That's it. So even if it's working, you're on a clock. Then what? You stop, the appetite suppression fades, and the weight comes back. I've seen it happen over and over.
GLP-1s don't have that arbitrary clock. We can titrate up, adjust doses, and keep patients on a sustainable path for months or even years. That's the difference between a jumpstart and a real plan.
Phentermine still has a place for the right patient — someone who needs a lower-cost, short-term option. But if you're dealing with chronic weight regain, it's usually not the strongest long-term strategy.
