You Hit Your Goal Weight — Now What? Your Guide to GLP-1 Maintenance

Hi everyone,


One of the most common questions we get at the clinic — and honestly one of the most important — is: "What happens when I hit my goal weight? Do I just... stop?"


The short answer: no, you don't just stop. And here's why that's actually great news.


The Good News First


Here's something we love seeing at our clinic: some patients do successfully titrate all the way down and come off the medication entirely — and they keep most of their weight off. How? During their time on Semaglutide or Tirzepatide, they genuinely relearned how to eat. Smaller portion sizes became their new normal, not something they had to fight for. And maybe even more importantly, they're now far more physically active than they were before losing the weight. When you're 40, 50, 60+ pounds lighter, you move more — you want to move more — and that extra activity burns significantly more calories day to day, helping keep the weight in check without the medication.


That said, not everyone's journey looks the same, and stopping abruptly without a plan is where things can go wrong.


Why Abruptly Stopping Is Risky


A major meta-analysis published in eClinicalMedicine found that patients who abruptly discontinued their GLP-1 medication regained an average of 12.4 lbs, with blood pressure, blood sugar, and cholesterol levels all rebounding toward pre-treatment levels. Separate research from the University of Oxford showed that patients who stopped semaglutide or tirzepatide returned to their baseline weight in as little as 18 months — gaining back roughly 1.8 lbs per month.


That's exactly what our maintenance protocol is designed to prevent.


Our Step-Down Approach


Rather than stopping cold turkey, we gradually reduce your dose in a controlled, monitored way:

  • Weeks 1–4: Step down to about 2/3 of your current dose. We monitor hunger, cravings, and fullness.

  • Month 2: Reduce further to approximately 1/3 of your peak dose — this becomes your long-term maintenance dose for most patients.

  • Ongoing: Regular check-ins to fine-tune. We may adjust slightly up or down based on how your body responds.


Most of our patients find their "sweet spot" at roughly one-third of their highest weight-loss dose. Just enough to keep the food noise quiet while you live your normal life.

Your maintenance dose is a powerful tool, but it works best alongside the habits you've built:

  • Prioritize protein and fiber at every meal

  • Stay hydrated — thirst is often mistaken for hunger

  • Move your body — whatever you enjoy, just keep moving

  • Reach out early — if cravings creep back or the scale starts moving, don't wait. Early adjustments prevent setbacks


The Bottom Line


A recent Cleveland Clinic study of nearly 8,000 patients found that among those who had structured follow-up care after stopping or reducing their GLP-1 medication, 45% maintained or continued losing weight a full year later. The difference-maker? Ongoing clinical support — not going it alone.
